Those who are eligible for influenza vaccination for the elderly or pneumococcal vaccination for the elderly and who are receiving support benefits such as those who are on welfare and Japanese who remain in China or who have a municipal tax including themselves Inoculation costs are free for all “tax-exempt households”.

How to proceed
* The long-term care insurance premium decision notice will be sent every year in mid-June, so if you want to receive the vaccination by mid-June, please use a copy of the previous year’s notice.* One vaccination co-payment exemption confirmation application and consent form is required for each person.

We will investigate based on the submitted “application and consent form”, and if you are a tax-exempt household, you will receive a “confirmation of exemption from self-payment”, and if you are not a tax-exempt household, you will receive a “confirmation of self-payment”. Will be sent.
The “confirmation letter” and “notice letter” will be sent to the address of the applicant (target person) . If you wish to send a “confirmation letter”, etc. to a place other than the applicant’s address, when submitting the “application form and consent form”, the applicant’s power of attorney (“receipt of confirmation letter, etc., ○ Please attach the content (delegate to Mr. ○).
