important point

For those who come by the second inoculation
① Both the first and second inoculations require an inoculation ticket (original) and identity verification documents.
If you have one, please bring a pre-examination slip as well. In particular, if you forget your identity verification documents, you will not be able to inoculate.
Also, if you forget your vaccination ticket at the time of the second vaccination, you will not be able to issue a vaccination certificate (certificate of vaccination), so please be sure to bring it with you.
② Please come to the center at the reserved time.
If you arrive earlier than the reserved time, you will have to wait outside the inoculation site until the reserved time.

Reservation method / reservation confirmation

Please make a reservation by entering the necessary information from the bottom of this page from the Internet.

(1) Enter the city code (6 digits), vaccination ticket number (10 digits), and date of birth, and click the “Next” button.
　* If the vaccination ticket number is less than 10 digits, enter “0 (zero)” for the number of missing digits at the beginning, and then enter the vaccination ticket number. (Example: If the inoculation ticket number is “123456789”, enter “0123456789”.) (
2 ) Check the inoculator information and terms of use.
* If you have entered the inoculator information incorrectly, click the “Back” button on your browser or “Deauthorize inoculation ticket” at the top of the screen and enter it again.
③ If the inoculator information is correct, click “Confirm input”. ④ Make a final confirmation and click “Proceed to reservation screen” to proceed to the reservation screen.
* Please note that the inoculator information cannot be modified after proceeding to the reservation screen. If you have any questions, please contact us.
⑤ Please make a reservation for the first vaccination at your convenience.
* The date and time of the second inoculation will be announced at the venue on the day of the first inoculation.
* After the reservation is completed, you can check or cancel the reservation status on My Page.
* To view My Page, please enter the required information again, click “Next”, and click “Click here to confirm / change / cancel your reservation”.
If you want to change it, please cancel the reservation and make a reservation again.
If you would like to change the reservation date, please contact the dedicated inquiry / reservation window.

City code	Please enter the half-width “6 digits” for the city code. If you do not see the vaccination ticket number, please check the website of the Ministry of Internal Affairs and Communications below. Click here to check the city code
Inoculation ticket number	Please enter the half-width “10 digits” for the inoculation ticket number. Please be sure to enter it without any hassle. An inoculation ticket issued by the target municipality is required.
