Application for exemption from expenses such as cancer screening
For those who are eligible, screening costs such as cancer screening will be free if you apply before the consultation . Even if you are applicable, you will be charged if you receive a medical examination without applying. The examination fee cannot be refunded. Please be sure to apply before the consultation.
All people over 70 years old (only breast X-ray examination for lung cancer screening over 65 years old) are free of charge, so no application is required.
Persons eligible for examination fee exemption
Those who fall under any of the following 1 to 3 will be free of charge for cancer screening and other screening fees by applying in advance.
1. Those who are receiving welfare benefits
2. Those who are exempt from municipal tax (annual tax amount of 0 yen) for all households including the person
3. Those who are covered by the support benefit system for Japanese remaining in China

3. Certificate
* Documents required for certification differ depending on the target person.
(1) Those who are receiving
welfare benefits ・ Certificate of welfare benefits (can be obtained at the window of each ward’s Social Assistance Division)
(2) Those who are eligible for the support benefit system for Japanese remaining in China・ Certificate of welfare benefits

(3) Those who are exempt from taxation (annual tax amount of 0 yen) for all households (including children) including the person himself / herself

If all households agree that Chiba City will investigate and inquire about the taxation status of the household, you can apply without the certificate, but please note the following points.
・ In the consent column on the back of the “application form”, consent from all households must be self-signed or stamped.
・ It may take more than 2 weeks to decide the exemption.
・ Depending on the results of the survey / inquiry, you may not be exempted.
