2. The “Chiba Prefecture> Chiba City” page will be displayed in the “List of Inoculation Venue”, so scroll down.
3. Please click “Read more” in “Notice from Chiba City ” to check the latest information. After confirmation, scroll down.
4. Set the search conditions. Press
   “Vaccine Maker” and select “Pfizer” “Takeda / Moderna” .
   * At this time, only Pfizer or Takeda / Moderna vaccines are available at individual inoculation venues in Chiba City.
   Make sure that “Display only available venues” is checked . Check
   “Display only venues that are generally accepted” when you want to search for medical institutions that are generally accepted and can be booked. If you want to search for medical institutions that can make reservations, including medical institutions that do not accept general acceptance, such as only family members can make reservations, uncheck it.
   If there are conditions you want to narrow down , enter the search conditions (medical institution name, ward name, address, zip code) in “Enter keyword” .
5. Check the set search conditions and press “Refine by this condition” .
6. A list of search results is displayed.
   Reservation acceptance status is indicated by a symbol to the left of the medical institution name.
   
7. If you press the “+” open button to the right of each medical institution name, notifications from the medical institution (reservation acceptance target person, reservation start date, precautions, etc.) will be posted, so please check.
   In addition, contact information will be displayed only for medical institutions that can make online reservations and telephone reservations. Click the “Go to Reservation Page” button
   under “Reservation on the Internet ” to move to the reservation page. When making a reservation by phone, please confirm the phone number and reception time, and be sure to call within the reception time. Also, please be careful about mistaken calls.
